When a patient is added or updated in MatrixCare, that information automatically syncs with the CharmHealth EHR. The consulting provider can review the patient record, document their interaction, and when the chart note is signed, it flows back to MatrixCare as a Progress Note. There are no duplicate entries, and no one is forced to chase down records. Everything is there, just like magic.

Practically speaking, this means patient records stay consistent. Records sync in real time, so providers are always working with complete, current information. And the clinical documentation travels with the patient. This ensures that everyone from the outpatient and long-term care teams works from the same data.
